The Health Information Management Department consists of these functional areas:
- Release of patient information (Guidelines)
- Patient Registration
- Accessibility and Diversity
- Provision of statistical information
- Provision of information for patient care
- Medical transcription
Each functional area supports and contributes to patient care.
Guidelines for Access to Patient Information :
To access patient information in accordance with legislative requirements we require a written authorization containing the following:
- The authorization must be directed to The Credit Valley Hospital
- The name of the patient
- Patient's date of birth
- Reason for the request
- The type of information required and treatment dates if possible
- The name and address of the person you wish the information sent to
- The authorization must be signed, dated and witnessed
The authorization must be signed by the patient/substitute decision maker. An administrative fee will apply if the requested information is being used for non clinical reasons. The authorization is valid for three months.
