Complex Continuing Care is provided for long term care residents who have chronic conditions requiring active medical management, frequent medical interventions and technologically based multidisciplinary hospital care. These residents will need extensive assistance with activities of daily living and will often have complex medical, mental and functional problems.
Our unit also services stable, long term palliative care patients who require symptom/pain management that exceeds the capability of a long term care facility or home environment.
Services
Complex Continuing Care is provided on a permanent basis unless the patient’s care needs change. There is a Long Term Care co-payment which is based on the patient’s income and assessed at time of admission and each subsequent year.

Why is there a co-payment?
Copayment fees are determined and set by the Ontario Ministry of Health and Long-term Care. Credit Valley Hospital must charge the Co-payment fee just as all Ontario chronic care facilities do. The co-payment is the resident’s contribution toward accommodation and meals. The co-payment is charged to a patient whose doctor has determined required complex continuing care and is more or less a permanent resident in a hospital or other institution. Waitlist, how long for admission process?
Referrals can be forwarded to the Social Work department and then are review by the Complex Continuing Care Admissions Committee. The length of time for the admission process is dependent upon bed availability.
Is a medical referral necessary?
Yes, Please complete the OHA standard forms for Complex Continuing Care.
Are tours available?
Yes, we do offer tours of our facilities. Please contact the Social Work department at 905.813.2399.
- Applicants must be over the age of 18 years. Those under 18 years will be considered in special circumstances.
- The diagnosis must be established and the medical condition should be stable.
- It must be demonstrated that the patient has care requirements that exceed the capability of a long term care facility.
- Only those applicants with a physician who has admitting privilege at Credit Valley Hospital will be considered.
- Those applicants living in the Credit Valley Hospital catchment area and/or have family living in the catchment area are prioritized.
- The waitlist is updated and prioritized on a regular basis by the admission committee.
How much Therapy is available?
As the Complex Continuing Care Program is not a rehabilitation program, physiotherapy and occupational therapy service are limited. Rather the focus of this program is to help each patient achieve and optimal quality of life. We do this, in part, by providing patients with therapeutic and leisure activities.
