Edmonton Symptom Assessment System (ESAS)
We want to know how you are feeling when you come to the Cancer Centre for your appointment. By telling us how you are feeling each time you visit, your health care team will be able to help you manage your symptoms. The Edmonton Symptom Assessment System, or ESAS, will provide us with information about your symptoms..
What is ESAS?
ESAS provides a review of how you are feeling based on nine different symptoms.
It is the standard symptom screening tool in all of Ontario’s regional cancer centres.
Why should I complete my ESAS?
You and your health care team are partners in your care. By completing the ESAS, you will be able to help us understand how you are feeling and we will be able to identify problems and offer help as soon as possible.
When do I complete my ESAS?
You complete your ESAS as determined by the type of appointment you have at the beginning of your visit in the oncology clinics in Level 1.
How do I complete my ESAS?
- If you are visiting the cancer centre for the first time, a volunteer will hand you the paper form of ESAS along with other documents as a standard procedure.
- If you have been to the centre before, upon registration you will be asked to proceed to the new ESAS kiosk station where a volunteer will help you in completing a simple computer touch screen kiosk instead of paper.
- Give your ESAS form or kiosk printout to the health care provider when you have your appointment.
Your symptoms are important to us. Only you can tell us how you are feeling.
