Edmonton Symptom Assessment System (ESAS)

 

We want to know how you are feeling when you come to the Cancer Centre for your appointment. By telling us how you are feeling each time you visit, your healthcare team  will be able to help you manage your symptoms. The Edmonton Symptom Assessment System, or ESAS, will provide us with information about your symptoms.   

What is ESAS?

ESAS provides a review of how you are feeling based on nine different symptoms.

Why should I complete my ESAS?

You decide whether you want to complete the ESAS or not. You and your healthcare team are partners in your care. By completing the ESAS, you will be able to help us understand how you are feeling and we will be able to identify problems and offer help as soon as possible.

When do I complete my ESAS?

You complete your ESAS as determined by the type of appointment you have at the beginning of your visit in the oncology clinics in Level 1.

How do I complete my ESAS?

  • When you register for your appointment, a volunteer will hand you the form.
  • Circle the numbers that best describe how you are feeling at that time.
  • A score of zero means you do not have the symptom, a score of 10 means you have the symptom at its very worst.
  • Any other problems can be written under the last choice “Other Problem”.
  • Give your form to the healthcare provider when you have your appointment.

Your symptoms are important to us. Only you can tell us how you are feeling.
